License Restoration

If your DMV Compliance Summary indicates that you need to petition the Circuit Court in the jurisdiction where you reside, and you reside in one of the following counties, you will need to contact the Southwest ASAP office to schedule your evaluation:

• Buchanan County
• Dickenson County
• Lee County
• Russell County
• Scott County
• Tazewell County
• Wise County

Upon enrollment, the evaluation fee is $175.00, payable to the Southwest ASAP office.

Once you are enrolled, you will receive an enrollment packet from our office. This packet MUST be completed and returned on the day of your appointment.

Failure to complete and return the packet may result in your appointment being rescheduled and could delay your court hearing.
